Purpose
The purpose of this form is to collate identification details of all parade attendees. This information will be passed to Base Security. On arrival to the Base, you will be required to show identification. If you do not have this on the day, you will not be permitted entry until the appropriate checks can be carried out. This will cause lengthy delays.
This form is intended for members who have retired from the ADF. Serving members will be provided alternate means to relay their expressions of interest to attend.

If you or anyone in your party were present at the 1986 parade, please indicate as such with regards to the role filled on that day.
There is only room to provide details for 5 members in your party, including you. If you have more people in your party, you will need to return to this form to submit their details.
